Before getting my Chiropractic degree, I worked many years as a workout physiologist and director of a few cardiac rehabilitation programs. One thing I learned fairly quickly during that time is that what I thought was common sense about wellness as well as heart health specifically, actually wasn"t so common.

Abbreviations - If you"re audited, do not count on a chiropractor reviewing your records who"ll understand what "PR-L" or "ASRA" or even "Sublux" means. For that matter, even a non-chiropractor may have problem with non specific abbreviations like "PT." Did you send the patient for Physical Therapy or part time work? The auditor is not going to try to interpret your abbreviations, especially if a key isn"t absent that describes them. A wonderful SOAP note with one unidentifiable abbreviation can cause you to fail your audit. Therefore, make sure you are spelling it out clearly (another edge to EMR, push the button and the word comes out!) or that you provide a key that certainly interprets all your abbreviations.
